Facial feminization involves surgical and nonsurgical procedures to transform male features to be more feminine. Facial feminization is not just a single procedure, but rather can involve multiple procedures on fat, skin, muscle, and bone as appropriate. As everyone is unique, plastic surgery such as facial feminization is specific and tailored for each individual.

Jaw reduction is one aspect of feminizing a face from male to female. Jaw reduction can be surgical or nonsurgical based on the underlying anatomy and a person's preference.

Nonsurgical jaw reduction involves relaxer injections to shrink a large or wide masseter muscle. Relaxer treatments have minimal downtime and slim the face gradually over time. Relaxers have the added potential benefit of reducing facial pain and headaches in appropriate patients.

Bone reduction of the lower jaw bone (mandible) may be necessary for a more feminine appearance. Feminization of the lower jaw to reduce bone can create a more feminine oval or heart-shaped appearance. Bone reduction surgery has significantly more downtime, as compared to nonsurgical jaw reduction, but can create a greater degree of change in appearance.

This Washingtonian had started the transition from male to female facial appearance to match her gender identity. She consulted facial plastic surgeon Dr. Chaboki to review potential options. He reviewed various surgical and nonsurgical treatments, such as lip enhancement, cheek augmentation, and jaw reduction.

She had a wide, strong jaw which is more common in a male face. Prior to treatment, she also had a more square face. She underwent nonsurgical jaw reduction with relaxer injection (ex. Botox®, Dysport®, Xeomin®, Jeuveau™). Her face is softer and has a more oval appearance. She is happy with nonsurgical treatments and continues with Dr. Chaboki for continued feminization procedures.
